Provide Case Management Services which consists of: Assessment of individuals current status (shelter, income, health, mental health, employment, food, clothing, social/support network, finances, ADLs, etc), referral to necessary sources, linkage to appropriate community partners, coordination between agencies and services, monitoring of individual to assist with stability in the community. Assist individual with maintaining a fulfilling life, maintain independent living in community, and provide individual with coping skills/skill building to stay out of the hospital. Assist individual developing a Wellness Recovery Action Plan and Safety Plan. Psychosocial Rehabilitation Individual: Assist the individual with rehabilitative skill building, the personal development of environmental and recovery supports, learning skills to promote individual's self-access to necessary services, and support the emotional and functional improvement of the individual. Provides Community Transition planning if an active individual on current caseload is admitted to a psychiatric hospital. Provide close follow up of individuals that are high utilizers of the hospitals/ER/Jail, 30 day readmits, Olmstead, Conditional Release, Outpatient Commitments. Completes ISP Reviews on each individual on caseload every 30 days. Caseload management including discharges, engagement of individuals, requesting appointments to be scheduled, sending letters, phone calls etc. Responsible for ensuring that all services are authorized. Referring clients to internal and external services to meet identified needs. Collaborative documentation with the individual. Majority of documentation completed during sessions with the individual. Ensures individuals are seen per utilization guidelines and per the designated frequency as stated on the treatment plan. Completes all documentation using the electronic medical record. Link and/or serve as advocate with other agencies to garner needed resources for individuals. Involving and educating family members and other natural support systems to the extent possible, providing crisis intervention services as scheduled. Develops individualized and comprehensive plan of care based on assessment, individuals level of need and functional ability. Transports individuals as needed. Attends community meetings as assigned and necessary, Provides community outreach as necessary/assigned. Deals calmly and effectively with high stress situations. Is open to change and new information; adapts behavior or work methods in response to new information, changing conditions or unexpected obstacles; effectively deals with ambiguityQualifying factors include: High School Diploma/GED and 2 yrs. Experience in Social Services related position or Bachelor's Degree in Social Services or related helping field preferred. Experience in case management, mental health and or substance abuse preferred.
