To provide evaluative, consultative and treatment interventions to patients referred by a licensed physician. To maintain effective communication with center/agency staff, patients, families, and management..
Essential Skill/Knowledge FunctionsAssessment, evaluation and treatment of referred patientsAppropriate and timely development of Plan of Care of patients servedAppropriate and timely documentation of progress towards goals as outlined in United Rehab Policy and Procedure manualAppropriate and timely communication of patient status to members of the Interdisciplinary treatment teamAppropriate referrals to members of the Interdisciplinary treatment teamSupervision of all clinical activities of the Licensed Physical Therapy Assistant according to State Practice Acts.Provision of formal and informal feedback to Licensed Physical Therapy Assistant according to State Practice Acts.Supervision of Rehab AideOrders and/or assists in obtaining patient equipment and/or supplies to attain patient-related therapeutic goals.May modify equipment, appliances and/or splints to meet patient-related therapeutic goalsParticipates in conducting in-services to rehab and/or center staff regarding Physical Therapy interventions.Submits billing, payroll, and expense documentation in a correct and timely manner.Essential Administrative FunctionsParticipate in center/agency surveys (Licensure / JCAHO) and any subsequently required reports.Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.Attend and participate in mandatory in-services.Honor patients/residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.Comply with corporate compliance program.Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.Follow established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.Follow established safety procedures when performing tasks and/or working with equipment.Perform other related duties as necessary and as directed by supervisor.Licensure/Certification/Education RequirementsGraduate of CAPTE accredited school.
